Responsibilities:

• Home Visits: Build trusting relationships with unique bonds through regular home visits.
• Comprehensive Assessments: Evaluate physical, psycho-social, and environmental needs in the comfort of the patient's home.
• Education & Communication: Empower patients and families with knowledge and recommendations to enhance their quality of life.
• Team Collaboration: Work cohesively with other healthcare professionals to deliver top-notch care.
• Documentation & Supervision: Ensure timely paperwork submission and provide guidance to aides for optimal patient care.

Qualifications:

• Nursing Degree: From an accredited institution (Diploma, Associate, Bachelor's, or vocational nursing).
• Active RN License: Current state license as an RN.
• CPR Certified: Must provide proof of current certification.
• Reliable Transportation: Current driver's license and car insurance.
• Soft Skills: Compassionate, excellent communicator, friendly demeanor
• Experience: 1 year RN home care experience (Preferred)
