Licensed Clinical Care Advisor Transition of Care (Region 3)

Licensed Clinical Care Advisor Transition of Care (Region 3) Location: North Carolina – Region 3 counties: Alamance, Caswell, Chatham, Davidson, Davie, Durham, Forsyth, Guilford, Orange, Person, Randolph, Rockingham, Stokes, Surry, Yadkin. Travel within the assigned DSS Region is required. Field-based role: will interact with patients, members, or providers in person four to five days per week; work virtually from home when not in the field. Job Overview The clinical care advisor coordinates operations and workflows related to case management activities in support of specialty programs, such as Foster Care. The role includes coaching, case management via phone or home visits, and managing overall healthcare costs for the designated population through integrated physical/behavioral health case management and whole‑person care. Key Responsibilities Engage collaboratively with families, foster parents, and service teams to develop and sustain individualized treatment plans. Facilitate effective transitions of care for children and families moving between treatment settings. Conduct assessments to identify individual needs and develop comprehensive care plans. Support member access to quality, cost‑effective care and modify plans as needed. Coordinate with internal and external resources to meet identified needs, including integrated physical and behavioral health services. Address social determinants of health and incorporate them into care planning. Collaborate closely with various state agencies and maintain knowledge of the system of care philosophy and community‑based services. Build meaningful partnerships with designated populations and address cultural and linguistic needs. Evaluate health needs and identify applicable services and resources with members and their families. Provide patient education, medication reconciliation, and assist with arrangement of follow‑up care. Minimum Requirements MS/MA in social work, counseling, or related behavioral health field, or a degree in nursing, with a minimum of 3 years of clinical experience in social work counseling involving complex psychiatric and substance abuse treatment, or an equivalent combination of education and experience. Active, current, and valid license as an RN, LCSW (within state scope), LMHC, LPC (within state scope), LMFT, or Clinical Psychologist to practice within the applicable state or territory. Preferred Skills, Capabilities, and Experiences Travel to worksite and other locations as necessary. Experience with transitions of care in agencies serving children, youth, and families involved in Department of Social Services and Juvenile Justice. In-depth knowledge of therapeutic residential and facility‑based services in North Carolina, including crisis and transition homes, PRTF, high‑fidelity wraparound, and Assertive Community Treatment. Experience working with transitional age youth, young adults, and families served by Foster Care and Adoptive Assistance programs. Knowledge of resources required for safe community living, including LTSS, behavioral health, therapeutic, and physical health services. Service delivery coordination, discharge planning, or behavioral health experience in a managed care setting. Ability to coordinate service delivery and discharge planning across multiple agencies and providers.
