Key Responsibilities:

• Administer medications and treatments per physician orders 
• Monitor residents and report changes in condition 
• Complete accurate documentation and incident reports 
• Follow up on physician orders, labs, and care plans 
• Assist with admissions and service plan implementation 
• Supervise and support caregiver staff 
• Maintain compliance with WV assisted living regulations 
• Follow infection control and safety protocols 
• Communicate effectively with residents, families, and team 

Qualifications:

• Active LPN license in West Virginia (required) 
• Graduate of an accredited nursing program 
• Experience in assisted living or long-term care preferred 
• Ability to work independently and make sound clinical decisions
