Care Navigator: Reentry & Trauma-Informed for Men of Color
New North Citizens Council
New North Citizens Council in the greater Springfield area is seeking a Care Coordinator to provide case coordination and essential program services to men of color re-entering from incarceration. This in-person role centers on trauma-informed care and supporting recovery and reintegration.
The position emphasizes helping clients access training, employment, housing and social skills to facilitate successful community re-entry in Western Massachusetts.
