RN Case Manager

Role RN Case Manager responsible for providing end-of-life hospice care in patients' homes or community settings. Responsibilities Assess, plan, implement, and evaluate patient care; supervise hospice aide staff; coordinate medications and supplies; document clinical notes; participate in interdisciplinary team meetings; develop individualized care plans addressing patient needs and goals; ensure use of the 4Ms (What Matters, Medications, Mentation, Mobility) and age-friendly care. Qualifications and Skills Current RN licensure, valid driver’s license, and at least two years of RN experience, preferably in hospice or palliative care. Compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families during sensitive situations are essential. Key Duties Manage a patient/family caseload, handle on-call responsibilities, report activities to clinical leadership, maintain accurate documentation, collaborate with the team, and support patient dignity and comfort.
