Registered Nurse RN Resource Team

Position Overview Location: Chandler, AZ. Department: Clinical Care Operations (CCO). Shift: Night. Full‑time, 3/12‑hour shift pattern with weekend and holiday rotation. Weekend shift differential and float incentive available. This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients. Core Functions Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs; review patient history and physical with patient/family and other team members; review diagnostics and laboratory data and report abnormal results to the physician(s) and other appropriate caregivers. Formulate a plan of care, including the discharge plan, based on assessment data and patient/family/health team input; initiate a plan of care based on medical plan; revise the plan of care to reflect changing patient needs. Implement the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team; promote continuity of care by communicating patient status. Evaluate patient and family response to care and teaching; revise the plan of care as needed to achieve goals; evaluate patient progress towards goals and expected outcomes. Document assessment, planning, implementation and evaluation in the patient record in a timely, accurate manner; note physician orders accurately. Provide care based on the best evidence available; may participate in research activities within clinical practice. Contribute to society through activities that lead to excellent patient outcomes, participate in national nursing quality improvements, and professional organizations. Supervise staff and manage workflow of the department when required. Interact with all levels of staff, physicians, patients, families, external contacts, and clergy for spiritual support. Minimum Qualifications Must possess current, valid RN license in state of practice, temporary/interim RN license, or compact RN licensure. BLS certification is required. Experience in an acute‑care hospital setting within the past 12 months; one year of experience for registry/travel positions. Maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. For Home Health: ability to operate personal vehicle, possess a valid driver’s license, and maintain active auto insurance coverage. For behavioral health roles: possession of an Arizona Fingerprint Clearance Card and an Arizona Criminal History Affidavit at time of hire, maintained for duration of employment. Preferred Qualifications Bachelor’s degree preferred. Professional certification preferred. Additional related education or experience preferred.
