We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.Facility: BayCare HomeCareLocation: Tampa, FLStatus: Full TimeShift Hours: 8:00 AM - 5:00 PM / Monday – Friday / 1 Weekend Rotation a month.Home visits in assigned service area: TampaWeekend Work: OccasionalOn Call: YesBayCare offers a competitive total reward package including:Benefits (Health, Dental, Vision)Paid time offTuition reimbursementMileage reimbursement401k match and additional yearly contributionYearly performance appraisals and team award bonusCommunity discounts and moreThe Home Health PTA provides hands-on care in the home for a group of patients consistently in accordance with the principles and practices of physical therapy.Responsibilities include:Assists Physical Therapist with the administration of interventions utilized in physical therapy and rehabilitationMay progress patients in the treatment plan developed by the physical therapistCommunicates with all physicians involved in the plan of care and other healthcare practitioners (as appropriate) related to the current plan of careFollows established procedures, emergency and safety precautions at all times in the treatment of patients and use of modalitiesMaintains timely communications, records, reports, and other documentation in accordance with agency policies & procedures, and Medicare guidelinesCaptures documentation during the home visit following company policy using company issued laptopAssists in progressing therapy programs with prior assessments and approval, prepares clinical progress notes and participates case conference and utilization reviewsRequired Certifications and Licensures:Physical Therapist Assistant State of Florida LicensureDriver's License State of Florida (Florida driver's license is required within 30 days of hire)Basic Life SupportEducation:Associates Degree Physical Therapy AssistantExperience:2 year Physical Therapy Assistant experiencePreferred 1 year Home CareWhy join BayCare HomeCare?
