Position: Case Manager - Inpatient Adults RN

Shift: Day 5x8-Hour (08:00 - 16:30) - Monday to Friday

Job Description

We are seeking a dedicated and experienced Registered Nurse for the position of Inpatient Adult Case Manager. This role requires significant acute care experience and involves managing care for both adult and pediatric patients.

Key Responsibilities

• Provide inpatient care management services as the primary discharge planner for assigned patients.
• Manage a patient ratio of approximately 1:25.
• Collaborate with interdisciplinary teams, including Physical Therapy, Respiratory Services, Social Services, and Pharmacy.
• Participate in patient assessments and care planning.
• Ensure compliance with facility protocols and documentation requirements.

Requirements

• Experience: Minimum of 2 years of acute care case management experience, particularly in a large hospital setting. 6-12 months of specific acute care case management experience is mandatory.
• Certifications: BCLS certification required.
• Weekend Requirements: Availability to work every Saturday and Sunday on a 4x10 schedule for weekend shifts.
• Floating: Must be willing to float to different units as needed.
• COVID-19 Vaccine: Optional. Reporting of vaccination status required.

Facility and Patient Care Details

• Type of Facility: Level I Trauma, Teaching Hospital, Short-term Acute Care, Children's Hospital.
• Total Staffed Beds: 554.
• Patient Age Groups: Neonates, Infants, Adolescents, Adults, and Geriatrics.
• Daily Census: Approximately 500 patients.
