Job Summary

Caledonia Home Health & Hospice is seeking a compassionate and team-oriented Registered Nurse (RN) to provide comprehensive home health nursing services to patients of all ages. The RN will be responsible for patient admissions, assessments, evaluations, care planning, and ongoing nursing care in a home setting. Working collaboratively with an interdisciplinary team of healthcare professionals and paraprofessionals, the nurse will help ensure the highest quality of coordinated patient care throughout Caledonia and Southern Essex Counties.

Key Responsibilities

• Conduct patient admissions, assessments, and evaluations.
• Develop, implement, and update individualized care plans.
• Provide skilled nursing care in accordance with physician orders and agency policies.
• Monitor patient progress and document clinical findings accurately.
• Educate patients and families regarding health conditions, treatments, and self-care.
• Coordinate care with physicians, therapists, social workers, and other healthcare team members.
• Participate in case conferences and interdisciplinary care planning.
• Maintain compliance with state, federal, and agency regulations.
• Promote patient safety, independence, and quality of life.

• Current and active Vermont Registered Nurse (RN) license.
• Minimum one (1) year of recent clinical nursing experience.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.
• Experience in home health, hospice, medical-surgical, or community health nursing preferred.
• Valid driver's license and reliable transportation preferred for home visits.

Preferred Qualifications

• Previous home health or hospice experience.
• Knowledge of electronic medical records (EMR).
• Excellent patient education and care coordination skills.
