Physical Therapist Case Manager: Home Health

Full Time Physical Therapist Case Manager Anticipated Territory: Sacramento County: Elk Grove and Surrounding cities* Responsibilities Perform home evaluations and develop individualized care plans in collaboration with Physical Therapy Assistants. Assist patients with mobility, movement and regaining independence. Create and update detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality care. Evaluate and report patient progress to the supervising physician. Assist with documentation and administrative tasks as part of the concierge service. Create a flexible work schedule and manage time within targeted zip code territories. Maintain autonomy with no clock‑in/out system. Operate within the company’s technology stack, including provided devices. Receive mileage reimbursement at $0.70 per mile between visits. Qualifications Unrestricted California Physical Therapist license. BLS/CPR certification. Valid California driver’s license. Knowledge of local community resources. Understanding of individual development, human behavior and cultural influences on health care. Familiarity with state and federal home‑care regulatory guidelines (preferred). Experience in home health care (highly preferred). Experience with high acuity patients (plus). Experience working with interdisciplinary teams (highly desired). Case‑management fundamentals (highly desired). Self‑directed, empathetic, critical thinker. Outstanding interpersonal, leadership and organizational skills.
