Mission-Driven Culture: Connect with residents, families, and team members in meaningful ways Competitive Pay: $95,000 -$113,000/year credit given for experience Schedule: Full-time day shift with on-call weekend rotation required Leadership Opportunity: Play a key role in care transitions, outcomes, and rehospitalization prevention Recognition Program: Earn recognition for exceptional service through our Culture Program Structured Training: Comprehensive onboarding and ongoing development support What You’ll Do: Serve as the primary clinical coordinator for residents transitioning into and out of the the Long Term Care or Transitional Care Unit Oversee and manage clinical care plans to ensure optimal recovery and successful discharge outcomes Collaborate with physicians, therapy, case management, social services, and interdisciplinary team members Monitor resident progress and proactively address changes in condition to prevent rehospitalizations Educate residents, families, and staff on care plans, expectations, and recovery goals Support admissions, assessments, and care coordination for short-term rehabilitation residents Ensure compliance with Federal, State, and Local regulations, as well as organizational policies Participate in weekend on-call rotation to support clinical operations and continuity of care What You’ll Need: Current Illinois RN license required 2+ years of nursing experience in skilled nursing, transitional care, rehabilitation, or long-term care preferred Prior leadership, care coordination, or nurse manager experience preferred Strong clinical assessment and critical thinking skills Experience working with interdisciplinary teams and managing complex care transitions Ability to work independently and manage multiple priorities in a fast-paced environment Commitment to providing compassionate, resident-centered care Benefits Available to You: Health, dental, and vision insurance 403(b) retirement savings plan Daily Pay option Paid Time Off Tuition reimbursement program WellBeing mental health support 24/7 Chaplain counseling Structured career development through the 365 Program We are an Equal Opportunity Employer and are committed to a diverse and inclusive workplace.
