Psychiatric Nurse Practitioner Long-Term Care (Adults 18)

Our psychiatric nurse practitioners partner closely with each facilitys long-term care healthcare team to deliver individualized high-quality mental health care for residents. Responsibilities may include:

• Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
• Maintaining current patient records to clearly document problems treatment plans actions goals and changes as they occur.
• Educating and supporting caregivers and facility staff.
• Managing and adjusting psychiatric medications to promote stability and well-being.

Requirements for this position include:

• Graduate of an accredited Nurse Practitioner program.
• PMHNP license required.
• Experience in psychiatric mental and behavioral health services.
• Long-term care and/or geriatric experience strongly preferred.
• Outpatient and tele-psychiatry experience a plus.
• Willingness to travel within assigned service area.
