Growing together.As an RN Case Manager, you'll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient's needs.Primary Responsibilities:Performs patient assessments and collaborates with the care team to develop and implement a plan of careMakes referrals to other hospice care disciplines as needed or documents rationale for not doing soDevelops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policiesRecords pain/symptom management changes/outcomes as appropriateTeaches patients, families, and caregivers appropriate patient care techniquesRegularly communicates with clinical manager and care teamPerforms on-call responsibilities and on-call services to patients/families as assignedOther duties as assignedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current RN license, specific to the state(s) you are assigned to work1+ year of nursing experienceCurrent CPR certificationValid driver's license, reliable transportation and liability insurancePay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,540 to $137,310 annually based on full-time employment.
