Experience: Minimum 1 year of Home Health experience

License: State Physical Therapist License

Certifications: CPR Certification

Must-Have: One year of Home Health experience

Description: The primary goal of the Physical Therapist is to restore function to prevent or minimize disability following disease or injury. The Physical Therapist helps clients reach their maximum performance and function within their capabilities. This role involves evaluating and treating patients using medically prescribed physical therapy programs. The Physical Therapist may supervise Physical Therapy Assistants. They are responsible for making initial admission and evaluation visits. The Physical Therapist will notify the assigned Clinical Team Manager of clinical scores within established timelines. The ideal candidate will document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans. This individual will also perform other duties as assigned consistent with their skills and training. All activities will align with the organization's goals and mission.
