Floating Case Manager

As a nonprofit organization, we partner with individuals to promote healing, housing stability, and long-term wellness through compassionate, equitable support.If you’re passionate about community impact, we invite you to join our team.Purpose: (General description and summary)The Floating Case Manager serves as an interim support role within motel-based shelter programs, providing continuity of care in the absence of a permanent Case Manager and addressing day-to-day program needs as directed by the Director of Shelter Services. This position delivers outreach and supplemental support to participants, primarily adults with severe and persistent mental illness (SPMI), using person-centered, trauma-informed, and harm reduction approaches.The Floating Case Manager helps participants build skills, access resources, and navigate services to support progress toward stable housing. Responsibilities include, but are not limited to, skills training, crisis prevention and de-escalation, community integration, and comprehensive case management. This role may transition into a permanent assignment based on program needs.Accountabilities: (Responsibilities of the job)Supportive Skills TrainingProviding direct support in one-on-one settings within the assigned motel and community-based skills training.Provide individual and group skills training opportunities, including but not limited to ADLs, iADLs , coping, regulation, safety, communication , community navigation, etc. to assist in movement toward identified goals.Provide support in milieu setting as indicate d and opportunities arise.Engagement and CollaborationProvide a dvocacy, case management , and community navigation to support participants in working towards their identified goals .Willing ness to meet and engage with people “where they are,” including their environment and readiness for change.Meet consistently with participants within the motel setting and communityMaintain professionalism in encounters with participants and other providers.Must be able to assist in de-escalation and crisis as neededCase ManagementConduct outreach to participant roomsSupport participants in obtaining vital documents and navigate community systemsProvide transportation to essential appointments as indicatedAbility to work in collaboration with acute care settings if necessary.Provide support, assistance , and advocacy for linkage to resources and maintenance of resources.Coordinate with providers and resources involved in participant goals and care consistently, including but not limited to weekly contact and care conferences as indicated .Crisis Prevention/De-escalation/Harm ReductionUse a crisis prevention approach to decrease use of crisis services, Project Respond, EMS, police, hospitalization, etc.Ability to work in collaboration with emergency services ( ie . police, Project Respond, Multnomah County Crisis, Behavioral Health Unit)Must be able to assist in de- escalation and crisis prevention in community-based settings.Support harm reduction approach, linkage to resources such as, but not limited to clean needle exchange, NARCAN, MAT, SUD services, education around harm reduction, etc.Documentation :Complete documentation and data entry as required by contract and program needs ( ie . Electronic Health Rec ord, trackers , intake, discharge, etc.) within 48 hrs.Utilize centralized scheduling to account for participant appointment s , meetings, training, etc.Maintain a consistent work schedule, which may include weekends and holidays as required .Attend and positively participate in all require d meetings.Use a trauma -informed, person-centered approach and objective languag e in all forms of documentation.Description of other duties:Perform other duties as assigned by supervisor.Provide support to teammates as indicated , ie . Milieu support, front desk support, environmental safety, etc.Core Competencies:Communication : Clear, direct, and concise. Keep people informed. Listen effectively to others.Teamwork : Effective at working in team situations.Utilizing available resources for personal wellness : Communicates with Supervisor or other designated New Narrative personnel to meet personal wellness and safety needs.Adaptability : Ability to modify or change service delivery as needed to best fit the needs of program participants and New Narrative valuesAnalytical thinking : Attention to detail over sustained periods of time.Dependability : Counted on to follow through on commitments: at work each day and on time; work completed in a timely fashion.Aptitude : Ability to comprehend and apply new ideas , processes and procedures, or training.Demeanor : Remaining calm in crisis situations , empathetic, unbiased, non-judgmental and excellent peer model skills.Supervision Received :Receive regular supervision from the Motel Program Manager , seeking additional supervision whenever necessary.Participate in interdisciplinary team meetings and group supervision as required .Other General Expectations:Uphold New Narrative Mission and Values in all interactions.W ork as a team member, remain flexible, and establish priorities as a group.W ork independently when necessary.R eceive direction from supervisor and engage in collaborative dialogue.Ability to learn and utilize new skills .Ability to display a non-judgmental and person-centered a pproach .Demonstrate initiative with daily shift dutiesAbility to multi-task and remain calm in a busy and stressful environment.Proficiency in Microsoft Office software (Word, Excel, PowerPoint, and Outlook)Follow all New Narrative policies, procedures, and protocols.Follow through on all directives and instructions.Maintain CPR and First Aid Certificate.Must be able to pass a criminal history check through the Department of Human Services.Qualifications:QMHA not required , but preferredCADC preferred, but not requiredMinimum two years of experience working with high acuity populationMinimum two years working in community-based settings preferredIncumbents with a substance use disorder must be sober and free of substance use for one-year preceding date of hire and must maintain this sobriety.Must maintain current Oregon driver’s license and insur ance.Must have a high school diploma or equivalent.All of our job openings are located in Portland, Oregon, and the surrounding metro areas. #J-18808-Ljbffr New-Narrative
