Responsibilities

• Perform patient assessments by taking vital signs, recording medical histories, and conducting physical examinations using EMR (Electronic Medical Record) systems such as Epic, Cerner, or Athenahealth.
• Administer injections, medications, and IV infusions while adhering to HIPAA regulations and sterile techniques.
• Assist with diagnostic evaluations including specimen collection, processing, and basic medical imaging procedures.
• Support acute care needs such as airway management, ventilator operation, and acute pain management in hospital or emergency room settings.
• Conduct triage by evaluating patient symptoms to determine urgency levels for urgent care, emergency medicine, or primary care visits.
• Manage patient documentation accurately, including medical records, ICD coding (ICD-9/10), CPT coding, and detailed medical documentation of treatments provided.
• Collaborate with multidisciplinary teams in various environments such as long-term care facilities, assisted living communities, nursing homes, or hospital units like ICU or PICU.
• Assist with specialized procedures including catheterization, suturing, spinal taps, or dermal fillers when applicable.
• Provide compassionate care for individuals with complex needs such as dementia, Alzheimers disease, memory care patients, or those with infectious diseases requiring infection control protocols.
• Support discharge planning and case management efforts to ensure seamless patient transitions across care settings.

Qualifications

• Valid LPN license or CCMA certification required; additional certifications such as Basic Life Support (BLS) are preferred.
• Demonstrated experience working in diverse healthcare environments including hospital medicine, emergency rooms, ICU/trauma centers (Level I/II), nursing homes, or outpatient clinics.
• Proficiency with EMR/EHR systems like Epic, Cerner, eClinicalWorks or Athenahealth for accurate documentation and coding.
• Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques.
• Experience with specialized patient populations such as pediatrics, geriatrics, neonatal ventilator management or individuals with disabilities is highly valued.
• Ability to perform venipuncture (venipuncture), phlebotomy, catheter care and sterile processing techniques confidently.
• Excellent communication skills to effectively triage patient needs and collaborate within multidisciplinary teams.
