The Optimal team embodies compassion, heart, patience, empathy, and competence to meet our patient’s needs.Core values: Commitment Advocacy Respect ExcellenceTHE OPTIMAL HOSPICE MEDICAL SOCIAL WORKER:The role of the medical social worker includes encompassing emotional support to patients and families, resource referral, advocacy, crisis intervention, educating families and patients on hospice and resources, assessing patients, and creating a care plan.Develop a plan of care in collaboration with the interdisciplinary team in order to meet the needs of the hospice patient and family.Assist hospice team members in understanding the social and emotional factors related to the hospice patient’s terminal illness.Provide emotional counseling to the hospice patients/families and assist patients/families in coping, decision making, advanced directives, funeral planning, or facility placement.Conduct assessments of hospice patients’ and families’ psychosocial, financial, environmental, and community needs.Makes appropriate referrals to community resources based on the assessment of the hospice patient’s needs.Comply with applicable legal requirements, company standards, policies, and procedures.Complete all documentation in an accurate and legible manner.
