Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Provide home health physical therapy services to patients across the lifespan in Summersville, WV. Plan, implement, and evaluate individualized plans of care based on referral and ongoing assessment. Collaborate with an interdisciplinary team to restore or maintain patient function and independence. Document progress, revise plans of care as needed, and educate patients and families on safe use of durable medical equipment and transfer techniques.

Minimum Qualifications

• Current state license as a Physical Therapist valid in West Virginia. Required.
• Bachelor of Science, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association or equivalent. Required.
• Proof of current CPR certification. Required.
• Current driver’s license, reliable transportation, and insurance. Required.
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting. Required.
• Ability to perform job functions that include heavy lifting, bending, pulling, pushing, prolonged standing and walking. Required.
• Ability to work a flexible schedule and travel locally. Required.
• Must read, write, and speak fluent English. Required.
• Previous home care experience. Preferred.

• Previous home health experience. Preferred.

Assignment Details

• Work involves travel to patient homes; local travel required.
