This open position is hired as an LPHA under Los Angeles County SAPC / DHCS Drug Medi-Cal Organized Delivery System rules. The LPHA establishes medical necessity, completes or finalizes ASAM multidimensional assessments, assigns DSM-5-TR SUD diagnoses, and reviews treatment plans so registered and certified counselors may deliver DMC-reimbursable services. As assigned, the LPHA also provides intake assessments on the Street Medicine Mobile Van team for unhoused and community outreach clients. This is not the Clinical Supervisor seat. Schedule is 30 hours per week, non-exempt. Hours on the van count toward the 30-hour week. This position reports to the Medical Director or qualified designee.

30% ASAM / medical necessity Interview, assess, and complete or co-sign ASAM multidimensional assessments at the clinic and at SAPC-required re-assessment intervals. Document medical necessity. Do not alter an ASAM to force a claim.

20% Street Medicine intake As assigned, ride the Street Medicine Mobile Van and complete intake assessments for persons served in the community, including unhoused clients. Same LPHA / Part 2 / ASAM standards as on site. Coordinate with CMA and medical staff on the run. Do not drive the van unless separately authorized and insured.

15% Diagnosis and MAT clinical Assign DSM-5-TR SUD and relevant co-occurring diagnoses within LCSW scope. Interview, assess, and provide clinical diagnoses for the MAT program with the Medical Director / PA / NP. Do not induct or change a MAT dose.

15% Treatment plans / LPHA review Develop, review, and LPHA-sign treatment plans on SAPC timelines. Review counselor work that requires LPHA co-signature. Return incomplete work to the Medical Director until a Clinical Supervisor is hired. Do not rewrite a counselor note to pay a claim.

10% Direct clinical services Individual and group SUD counseling / psychotherapy within LCSW scope. Crisis intervention on site or on the van.

10% Quality, privacy, timekeeping Sage / PCNX same day when possible. 42 CFR Part 2. Accurate non-exempt time record, including van hours and meal breaks. Overtime only with Medical Director / COO preapproval.
