Registered Nurse (RN) Discharge Planner
Talantage is seeking a dedicated Registered Nurse (RN) Discharge Planner to join our team at our client's correctional institution. The RN Discharge Planner is responsible for coordinating comprehensive discharge planning within a correctional healthcare setting to ensure continuity of care following patient release. Working under the direction of the Director of Nursing, the RN Discharge Planner collaborates with a multidisciplinary team to develop and implement safe, effective, and individualized discharge plans.
Key Responsibilities:
- Coordinate discharge planning for patients transitioning from a correctional setting to the community
- Assess patient medical, behavioral health, and social needs related to discharge
- Collaborate with nursing, medical providers, behavioral health staff, and community agencies
- Arrange follow-up care, medication access, and referrals to community resources
- Ensure discharge plans promote continuity of care and patient safety
- Maintain accurate and timely documentation in accordance with policies and regulations
- Comply with all facility security requirements and correctional healthcare standards
Qualifications:
- Current Registered Nurse (RN) license in the state of Maryland or ability to obtain licensure
- Current BLS and CPR certifications
- One (1) year of experience as a Discharge Planner or Case Manager required
- Experience in correctional healthcare or acute care preferred
- Ability to obtain and maintain security clearance, including drug screening and criminal background check
- Strong communication, organizational, and care coordination skills
