Social Worker LCSW - Social Work Services

Social Worker LCSW – Social Work Services (Night Shift, On‑call) The Licensed Clinical Social Worker (LCSW) will independently provide comprehensive biopsychosocial/spiritual assessment, diagnosis, and treatment/interventions/advocacy for patients and their support systems. The LCSW will collaborate with patients and support systems to develop and implement evidence‑informed, person‑centered care plans across the care continuum in collaboration with the Inter‑Multidisciplinary Team (IDT). Responsibilities

• Conduct comprehensive biopsychosocial/spiritual assessments for patients and families.
• Provide diagnosis, treatment planning, and interventions for mental‑health and social service needs.
• Advocate and coordinate care with multidisciplinary team members.
• Maintain accurate patient documentation and regulatory compliance.

Required Qualifications Master's Degree in Social Work from a CSWE accredited School of Social Work, or a school in candidacy status by CSWE. Oregon Clinical Social Worker License upon hire. Relevant social work experience (including CSWA). Job Details Schedule: Per‑Diem, Night Shift. Location: Providence St Vincent Medical Center – Portland, OR. Pay Range: $41.63 – $64.63 per hour (may include shift differentials, overtime, etc.).
