Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician directions. This role involves delegating and directing all discipline-specific therapy services for assigned patients while ensuring the design of functional treatment programs that facilitate effective community reintegration. The therapist also serves as an educator for patients, families, students, coworkers, and themselves, performing other professional duties related to patient care and the development of therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals to progress patients towards discharge.
• Document patient care accurately and timely in compliance with guidelines.
• Ensure care is provided under signed, valid physician orders.
• Participate in student programs by providing supervision and evaluations.
• Proactively pursue professional development through education and involvement in professional activities.
• Adhere to discipline-specific regulations and maintain professional licensure in good standing.
• Understand and contribute to performance improvement initiatives within the organization.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for practicing as a Physical Therapist.
