RiseWell Community Services is seeking a Full-Time Overnight Case Manager to work in the SOS Team in Brooklyn & Queens, NY. You will join the Overnight multi-disciplinary team to assess health and social needs of the homeless population, with late evening to early morning hours (including weekends and weekdays). Responsibilities include outreach on streets/subways across Brooklyn/Queens, coordinating referrals to overnight facilities, advocacy, and ensuring warm hand offs and follow ups with SOS #J-18808-Ljbffr RiseWell Community Services
