Seven Healthcare are seeking a dedicated Physical Therapist for a contract assignment in Covington, GA. This position focuses on providing physical therapy evaluation and treatment based on physician referrals within a home health care setting, offering an opportunity to work directly with patients to help them achieve their maximum level of independence.

- Review patient records for current diagnosis, history, precautions, and contraindications
- Establish measurable goals and develop treatment plans
- Regularly reassess the effectiveness of treatment plans and patient progress
- Communicate patient needs and progress to the treatment team and others
- Consult with medical providers regarding evaluations and treatment
- Recommend and assist in securing adaptive equipment for patients
- Maintain and submit required documentation
- Adhere to standards of business conduct and professional practice
- Supervise Physical Therapist Assistants and Aides
- Participate in or conduct staff development programs and obtain continuing education
- GA License
- Experience in Home Health Care
