RN Case Manager
MEDSTAR HEALTH
As an RN Case Manager at MedStar Health, you will lead safe, timely discharges and smooth transitions of care for diverse patient populations across our regional health system. Drawing on strong clinical judgment and case management expertise, you will assess patient needs, design individualized discharge plans, and coordinate services with physicians, nurses, social workers, and community partners. You will arrange post-acute care, educate patients and families, reduce readmissions, and support quality and patient-centered outcomes in a collaborative, mission-driven environment.
