On Call Hospice RN‑Nights Location: Bergen County We are seeking a compassionate and dedicated On‑Call Hospice Registered Nurse (RN) to join our team. This role is crucial in providing after‑hours care and support to hospice patients and their families. The ideal candidate will respond to patient needs during night shifts, ensuring high‑quality, end‑of‑life care in accordance with the hospice philosophy. Responsibilities Patient Care: Provide nursing care to hospice patients during after‑hours, addressing physical, emotional, and spiritual needs. Emergency Response: Respond promptly to patient calls, perform on‑site visits as needed, and manage urgent patient needs. Symptom Management: Assess and manage pain and other symptoms, providing comfort and relief to patients. Family Support: Offer guidance and emotional support to families during challenging times. Collaboration: Communicate effectively with the interdisciplinary hospice team, including physicians, social workers, and chaplains. Documentation: Maintain accurate and timely records of patient care and communication. Adhere to Best Practices: Follow VNA Health Group's hospice best practices, ensuring compassionate and patient‑centered care, effective communication, and continuous quality improvement. Qualifications Education: Current NJ RN license. Experience: Minimum of 1‑2 years of hospice or palliative care experience preferred. Skills: Strong assessment, critical thinking, and communication skills. Availability: Ability to work night shifts, including weekends and holidays as scheduled. Transportation: Reliable vehicle, valid driver’s license, and proof of auto insurance. Working Conditions / Physical Demand Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact. In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $89,700 to USD $97,800.
