CNA - LTC/SNF

Job Description

Job Summary:
Seeking a compassionate and detail-oriented CNA to provide direct patient care in a long-term care or skilled nursing facility. The ideal candidate will have at least one year of experience and be comfortable with a variety of clinical tasks, including patient transfers, hygiene, and vital signs monitoring.

Responsibilities:

• Assist patients with ambulation, transfers, and repositioning to promote safety and comfort.
• Perform bathing, dressing, and hygiene care, including bed baths, skin care, and oral hygiene.
• Monitor and record vital signs, including blood pressure, temperature, respirations, and pulse.
• Provide specific care such as foot care, incontinent care, and pressure injury prevention.
• Ensure infection control by following PPE and isolation protocols.
• Support documentation using electronic charting systems like EPIC, including module completion and specimen collection.

Qualifications:

• Possess a valid CNA license in the state of New York.
• Demonstrate at least one year of clinical experience in a long-term or similar setting.
• Be proficient in vital signs measurement and basic patient care procedures.
• Have experience with electronic health records, preferably EPIC.
• Capable of lifting and repositioning patients, ensuring safe body mechanics.
• Ability to work evening shifts (5x8 hours).
