Job Description\n CloudinfraIT Inc is seeking a travel Physical Therapist for a travel job in San Jose, California.Job Description & Requirements\n \n Specialty: Physical Therapist\n Discipline: Therapy\n Duration: 13 weeks\n 40 hours per week\n Shift: 8 hours\n Employment Type: Travel\n \n Travel Physical Therapist – California\n San Jose, CA\n 13-Week Contract | Day Shift | 40 Hours/Week\n We’re hiring an experienced Physical Therapist (PT) for a skilled nursing facility travel assignment in California!\n Requirements:\n • Minimum 3 years of Physical Therapy experience required\n • Active California Physical Therapist License\n • Graduate of an accredited Physical Therapy program\n • Ability to provide skilled PT services in a Skilled Nursing Facility (SNF) setting\n • Strong clinical assessment and treatment-planning skills\n • Experience developing and implementing individualized plans of care\n • Ability to evaluate patient mobility, strength, balance, and functional limitations\n • Skilled in therapeutic exercises, gait training, transfer training, and functional mobility\n • Strong patient-care, communication, and documentation skills\n • Ability to work independently and effectively as part of an interdisciplinary team\n • Must maintain accurate and timely clinical documentation\n • Commitment to patient safety, quality care, and professional standards\n Assignment Details:\n • 13-week contract\n • 40 hours/week\n • Monday–Friday\n • Day shift\n • Skilled Nursing Facility setting\n • 1 position available\n • No weekend requirement listed\n • No holiday coverage required\n • No on-call requirement\n • W-2 employment only\n Key Responsibilities:\n • Perform comprehensive physical therapy evaluations and assessments\n • Develop individualized treatment plans based on patient needs and goals\n • Provide skilled therapeutic interventions to improve strength, mobility, balance, and function\n • Perform gait and functional mobility training\n • Provide transfer and ADL training as appropriate\n • Monitor patient progress and modify treatment plans accordingly\n • Educate patients and caregivers on exercises, mobility, and safety techniques\n • Coordinate care with physicians, nurses, therapy staff, and other interdisciplinary team members\n • Document evaluations, treatment sessions, progress, and outcomes accurately\n • Ensure treatment is delivered according to the established plan of care\n • Promote patient independence and safe functional mobility\n • Follow infection-control, patient-safety, and facility policies\n • Maintain professional and compassionate patient interactions\n CloudinfraIT Inc Job ID #299266.003.
