Heartland Hospice is seeking a Hospice Care Consultant (HCC) to champion our mission and build lasting relationships with hospitals, home health agencies, skilled nursing facilities, clinics, and physician offices.

As a Brand Ambassador, you will educate healthcare professionals about hospice options, identify referral opportunities, and lead targeted outreach to increase admissions while representing Heartland Hospice at community events and professional associations; you will tailor
