Job Description for a Client Assessment Nurse:

We are currently seeking a PRN Client Assessment Nurse (RN) to join our team. The Client Assessment Nurse will be responsible for providing case assessment services to our clients, ensuring they receive the highest level of care and support. Responsibilities of a Client Assessment Nurse:

• Complete periodic assessments of home care clients per VA requirements. Visits will last from 1-2 hours and occur at intervals throughout a client's engagement.

• Travel independently to client's homes throughout the southern Denver Metro suburbs to provide assessments.

Client Assessment Nurse Skills and Qualifications:

• Current and valid RN license in the state of Colorado.

• Strong assessment, communication, and interpersonal skills.

• Ability to work independently and as part of a team.

• Compassionate and empathetic approach to patient care.

• Reliable transportation and willingness to travel to client homes as needed.
