Overview
Licensed Practical Nurse (LPN) – Long Term Care, Concord, NH. Travel temporary assignment starting 07/27/2026 for 13 weeks with 40 guaranteed hours per week. Evening shift (15:00–23:00), five days per week. Work in a long-term care setting delivering skilled nursing care to residents on a temporary basis.

Responsibilities
- Administer prescribed medications and monitor residents per care plans and physician orders
- Perform routine assessments, monitor patient status, and document changes accurately
- Collaborate with the interdisciplinary team to update and implement care plans
- Provide routine nursing procedures, wound care, and assist with activities of daily living as needed
- Ensure safety and quality of care, respond to patient calls, and maintain accurate charting and documentation

Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Graduation from an approved practical nursing program (LPN)
- Prior long-term care or skilled nursing facility experience preferred
- Ability to work evenings (3 PM–11 PM) with guaranteed hours
- Strong clinical assessment, medication administration, and documentation skills
- Current CPR/BLS certification or able to obtain prior to start
