Behavioral Health Technician

Oak Street Health is seeking a Behavioral Health Specialist to join our patient-centered primary care team. In this role, you will provide brief behavioral interventions, conduct psychosocial assessments, support care planning, and coordinate services for adults on Medicare in underserved communities. You will collaborate closely with primary care providers, social workers, and care managers to address mental health, substance use, and social determinants of health. The ideal candidate is compassionate, culturally sensitive, skilled in motivational interviewing, and passionate about preventative, community-based care and improving health outcomes. Responsibilities Conduct comprehensive psychosocial and behavioral health assessments for adults on Medicare Provide brief, evidence-informed behavioral interventions in a primary care setting Collaborate with primary care providers and care teams to develop and adjust care plans Identify and address mental health, substance use, and social determinant needs Coordinate referrals to community resources and specialty behavioral health services Monitor patient progress and document care in the Electronic Health Record (EHR) Participate in case conferences, huddles, and team-based care activities Support crisis intervention and safety planning when needed Educate patients and families on coping strategies, treatment options, and self-management Contribute to a patient-centered, inclusive, and outcomes-focused care culture Qualifications & Competencies Psychosocial assessment Brief behavioral interventions Cognitive Behavioral Therapy (CBT) basics Motivational interviewing Care coordination and case management Electronic Health Records (EHR) documentation Risk assessment and safety planning Crisis intervention Knowledge of Medicare population needs Interdisciplinary team collaboration Benefits Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensing & Certifications State licensure or eligibility as LCSW, LCPC, LMHC, LPC, LMFT, or equivalent Certification in Behavioral Health or Integrated Care (preferred) Training in Motivational Interviewing (preferred) CPR/BLS certification (preferred) #J-18808-Ljbffr
