Your Role

We are looking for an energetic, geriatric focused Physical Therapist to act as a partner to our senior patients throughout the trip of restoring motion and ensuring they function at their personal best. You will maximize the quality of life and movement potential within the spheres of prevention, diagnosis, treatment, intervention, and rehabilitation. We serve the entire Santa Clara County area and broader Bay Area. This position requires driving to patient’s homes.

Essential Duties and Responsibilities

• Identify and meet patients’ goals and develop a Plans of Care
• Offer cost-effective treatments that help improve clients’ motion and mobility
• Reduce the need for medications and provide alternatives to surgery
• Develop care plans using a variety of treatment techniques
• Create fitness- and wellness-oriented programs tailored to patients’ specific needs
• Provide quality, personalized and evidence-based care, and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
• Consult and practice with other health professionals
• Evaluate effects and monitor and communicate progress
• Document patient care services in Ready2Nurse's EMR System

Skills

• Proven work experience as a home health physical therapist
• Geriatric experience a great plus
• Intensive education and clinical expertise
• Ability to manage patients with different types of personalities
• Current knowledge of treatment practices
• Drive to continually learn and grow
• Professional behavior
• Excellent interpersonal communication skills
• Valid license to practice physical therapy
• Graduate degree or greater in physical therapy
• Minimum 1 year experience in Skilled Nursing Facility experience or Home Health experience preferred

Seeking Full Time Hire.

Physical Therapist's seeking Per Diem work will be considered. $75/hr

$10,000 signing bonus ($5,000 on the first day and $5,000 after 6 months on the job).

Job Types: Full-time, Per diem

Pay: $130,000.00 - $140,000.00 per year
