Therapist - Physical Therapist

Job Summary:

Provides skilled physical therapy services to patients in their homes, promoting recovery and improving mobility. Ensures compliance with clinical standards and maintains thorough documentation of patient progress.

Responsibilities:

• 
  Assess patients' physical conditions and develop individualized treatment plans.
  
  
• 
  Deliver therapeutic interventions during home visits, including corrective exercises and manual therapy.
  
  
• 
  Document patient progress accurately and submit reports within 24 hours of each session.
  
  
• 
  Coordinate with physicians and care teams to ensure optimal patient outcomes.
  
  
• 
  Manage a caseload of 5-6 patients daily, prioritizing care based on clinical needs.
  
  
• 
  Travel up to 75 miles from the home clinic, utilizing reliable transportation.
  
  

Qualifications:

• 
  Hold a valid state license as a Physical Therapist.
  
  
• 
  Minimum of 1 year of recent adult home health therapy experience.
  
  
• 
  Proficiency in HomeCare HomeBase EMR system.
  
  
• 
  Must have a current drivers license, reliable vehicle, and agency-required liability insurance.
  
  
• 
  Ability to travel and cover multiple locations within the territory.
  
  
• 
  Experience with OASIS documentation is preferred.
