Professional and Compliance Requirements: - Trinity Conf. of Patient & Pty. Info. & System Security Agmt. - Trinity Conflict Of Interest Disclosure Form - Trinity Criminal Background Check - Trinity Education Verification - Trinity EPLS - Trinity GSA/OIG - Trinity Integrity and Audit Services Form - Trinity Joint Commission Testing - Trinity NSO - Trinity System Access Form - Trinity Code of Conduct Form - Trinity I9 - Trinity MVR (Motor Vehicle Report) 3. Checklist/Testing: - Trinity Skills Checklist 4. Licenses and Certifications: - License/Certificate Verification Additionally, applicants must complete a Trinity Profile for submission. Influenza Vaccine is optional for onboarding. The role requires working 7:30-5 PM with 1 weekend a month and 4-5 nights per pay period including weekend calls.
