How Hiring: Care Coordinator

Highland Behavioral Health is seeking a compassionate and motivated Care Coordinator to support residents in achieving their individual goals and accessing the services and resources they need. This role provides case management, service planning, coordination with families and community providers, and direct support to residents.

What You'll Do:

• Provide case management and support to residents

• Assist residents with daily living skills and managing psychiatric symptoms

• Coordinate medical, social, educational, and rehabilitative services

• Develop and implement individualized service plans

• Collaborate with residents, families, staff, and outside providers

• Monitor resident progress and document service outcomes

• Prepare annual assessments, service plans, and progress notes

• Provide support during potential crises and develop appropriate interventions

• Transport residents to appointments and outings as needed

• Assist with group treatment and recovery-oriented activities

Qualifications:

• B.A. or B.S. in Social Work or a related field OR 2 years of experience working in mental health

• Strong communication, organization, and problem-solving skills

• Ability to coordinate multiple activities and meet deadlines

• Basic computer skills

• Valid First Aid certification
