Patient-centered independent outpatient primary care practice in Western New York is seeking an advanced practitioner. The practice currently consists of four family physicians, two nurse practitioners, six physician assistants and employs about 40 support staff.  Two locations - Westfield, NY and Sherman, NY. Although a largely middle class area, we also see uninsured and indigent patients, as well as nearly 15% Medicaid. Average number of patients: 16-20 per day.Westfield Family Physicians has a certified electronic health records (EHR) system that is fully integrated with its practice management system and its web-based patient portal. In addition, the practice is recognized by NCQA as a Patient-Centered Medical Home (PCMH). Committed to providing quality medical care while cultivating strong families ties and taking an active role in local schools and the community.
