Case Manager (RN) – Bronson Lakeview Hospital – PRN Responsibilities Move patients from admission through discharge with minimal disruption. Conduct assessment, planning, implementation, coordination, monitoring, and evaluation of patient caseload. Ensure appropriate care based on patient needs and hospital capabilities. Serve as an advanced clinical resource to patients, families, and staff. Collaborate with interdisciplinary teams to provide comprehensive, cost‑effective care. Monitor outcomes and participate in continuous improvement processes. Assess, prioritize, plan, organize, monitor, and evaluate patient needs and skill levels. Identify patients at risk for post‑hospitalization care, conduct additional assessments, and facilitate transition plans in coordination with the care team. Maintain communication with patients during the post‑discharge period. Conduct daily rounds on assigned patients. Identify readmissions, determine reasons, and implement interventions to prevent further readmissions. Drive multidisciplinary team rounds and document clear transitional plans. Collaborate with community resources to meet post‑discharge needs. Manage and coordinate discharge plans to achieve appropriate LOS and quality outcomes. Evaluate outcomes related to the case management process (LOS, readmission, patient satisfaction, financial metrics). Report variances and translate outcomes to reimbursement principles. Track and analyze outlier LOS data to reduce outliers. Support hospital operations by coordinating admissions, transfers, and patient placement. Assist unit leaders by providing clinical and administrative direction during their absence. Investigate and resolve safety concerns, patient or staff complaints, atypical events, and crisis situations. Coordinate admissions/transfers/patient placement and monitor hospital capacity with the staffing office. Conduct regular rounds of all hospital departments/units and maintain utilization review. Qualifications Licensed Registered Nurse in good standing with the State of Michigan. BSN (or higher) or expected to obtain one within 8 years of hire. Master's degree preferred. Minimum of 3 years of experience in an acute care hospital setting. Case Management Certification preferred. Proficient in word processing, spreadsheet, keyboard, presentation programs, and other relevant software. Ability to handle multiple priorities in a stressful environment. Effective verbal and written communication skills with all levels of healthcare providers. Ability to communicate in a patient‑ and family‑friendly manner. Critical thinking, decision‑making, problem‑solving skills. Strong negotiation, influence, and follow‑up abilities. Capacity to assess, prioritize, plan, organize, monitor, and evaluate patient needs. Independent, self‑motivated, effective conflict resolution skills. Physical ability to walk, stand, stoop, bend, sit, and lift light objects; ability to focus at a computer for 40‑70% of the time.
