Home Dialysis Nurse Educator & Case Manager
Fresenius Medical Care
Fresenius Medical Care is seeking a Home Therapy Registered Nurse to educate and support dialysis patients. You will train patients on peritoneal and home hemodialysis, and work primarily in the clinic with initial home visits as part of a patient care team. You will coordinate assessments, education plans, and interventions while collaborating with RNs, LPNs, and technicians to ensure safe, effective home dialysis delivery and patient outcomes. #J-18808-Ljbffr Fresenius Medical Care
