The Nephrology Nurse Practitioner (NP) plays a vital role in delivering comprehensive kidney care to patients with chronic kidney disease, dialysis needs, and acute kidney injury. Working across outpatient clinics, dialysis centers including in-center and home modalities, and hospital settings, the NP supports improved patient outcomes and streamlined care coordination in collaboration with nephrologists, primary care physicians, dialysis nurses, and multidisciplinary teams.

Responsibilities

• Conduct thorough patient assessments and manage chronic kidney disease treatment plans
• Coordinate care across outpatient clinics, dialysis units, and hospital environments
• Manage dialysis therapies including in-center hemodialysis, home hemodialysis, and peritoneal dialysis
• Perform hospital rounding duties daily, weekly, or as needed
• Manage referrals to ensure timely and appropriate specialist engagement
• Educate patients and families on disease management, treatment options, and self-care
• Prescribe and manage medications relevant to nephrology patients
• Support home dialysis patients and monitor treatment adherence
• Collaborate closely with nephrologists, primary care providers, and care teams

Required Qualifications

• Master’s or Doctorate in Nursing (MSN or DNP)
• Current Nurse Practitioner licensure in New Mexico or eligibility
• National board certification (AANP, ANCC, or equivalent)
• DEA license with prescriptive authority
• Entry-level experience in nephrology nursing

Preferred Qualifications

• Proficiency in patient assessment and chronic kidney disease management
• Experience with various dialysis modalities including in-center and home therapies
• Strong skills in care coordination, medication management, and patient education
• Familiarity with Electronic Health Records (EHR) systems
• Demonstrated critical thinking, communication, and team collaboration abilities
