Shift Options All shifts (1st, 2nd, and 3rd) Full‑time Part‑time Per diem Job Summary Provide 1‑on‑1 nursing care to patients in their homes, schools, or community settings, helping families navigate complex medical care and stay at home. Deliver continuous care nursing for medically complex children and adults. Manage a diverse caseload including feeding challenges, G or J‑tube feeding, tracheostomy/ventilator support, seizure disorders, ostomy appliances, wound care, and diabetic maintenance. Work collaboratively with interdisciplinary teams to ensure high‑quality, patient‑centered care. Pay and Benefits LPN hourly rates: $40 / $42 / $44 depending on differential.
