Physical Therapist (PT) or Physical Therapist Assistant (PTA) | Full-Time | Outpatient | Berea, KY

At H2 Health – Berea, you’ll join an experienced therapy team with more than 80 years of combined experience in a 2,500-square-foot outpatient clinic. The team serves a diverse general outpatient population and provides care across orthopedics, balance and dizziness, sports medicine, chronic pain, neurological conditions, workers’ compensation, and other rehabilitation needs. Specialty services also include manual therapy, ASTYM®, and cupping.

Your Role:

• Deliver individualized, evidence-based outpatient Physical Therapy
• PTs: Perform evaluations, develop plans of care, and guide patients toward functional goals
• PTAs: Provide skilled treatment under the direction of a licensed Physical Therapist
• Treat a varied caseload including orthopedic, balance, mobility, neurological, and musculoskeletal conditions
• Educate patients on movement, exercise, recovery, and injury prevention
• Collaborate with clinicians and support staff to coordinate high-quality care
• Complete timely and accurate clinical documentation

Requirements

Physical Therapist (PT):

• Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program
• Current Kentucky Physical Therapist license or eligibility for licensure

Physical Therapist Assistant (PTA):

• Associate degree from a CAPTE-accredited Physical Therapist Assistant program
• Current Kentucky Physical Therapist Assistant license or eligibility for licensure

For Both:

• CPR certification, if applicable
• Strong communication and teamwork skills
• Commitment to patient-centered care
• New graduates are welcome to apply; previous outpatient experience is not required
