For this position, Physician Assistants will manage patients in an outpatient clinic setting as well as providing in-home clinical support for patients in the community. GGH PA's will be supporting clients with their patient panels and partnering with clinics as an embedded member of the care team with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients’ medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare. Responsibilities Assess patients’ clinical problems and healthcare needs and provide clinical care and consultation Perform annual wellness exams for senior Medicare patients in a designated clinic or at their residence Obtain and record the patient’s medical history and symptoms, including medication reconciliation HCC coding, verification, and risk adjustment Coordinate continuity of care, prevention and avoidance of complications, and close clinical treatment and management under the direction of the patient’s primary healthcare provider(s) Perform various tests and screenings Counsel patients with education around their discharge plan Execute the discharge plan and use critical thinking to build an effective care plan during a critical transition after hospital discharge Support patients and their families, and provide compassionate care Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings Collaborate with care team to ensure completion of holistic patient care Travel within Jackson County, OR (within 30–35 mile radius) 5 days a week for in-home visits Complete charts within 72 hours of patients’ visits; have access to EMR Cover for other APPs and Care Team Members when necessary Maintain patient confidentiality and abide by HIPAA rules and regulations Qualifications Current board certified PA 2+ years of clinical experience as a NP in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care Acute care experience with high-risk patients is a plus Home health experience (PA or RN) is a plus Working knowledge of chronic care management and various medication options Current DEA licensure / Prescriptive Authority post-hire Active CPR/BLS Certification Demonstrate ability to execute thoroughly in a fast-paced work environment Build rapport with aging/elderly population Detail-oriented routine work Open to feedback for improvement Comfort with technology systems and timely documentation Flexible work schedule and setting as needed Benefits Competitive Compensation Package Comprehensive Medical, Dental, and Vision Benefits; Flexible Spending Accounts (FSAs) and Health Savings Accounts (HSAs) available Paid Time Off and Parental Leave 401K Program with Company Match Access to wellness resources and monthly reimbursement Basic Life, Accidental Death & Dismemberment (AD&D) coverage fully paid; voluntary supplemental life insurance offered at discounted rate Voluntary short-term disability, critical illness and accident plans License reimbursements for DEA, license, board certification, and ALS/BLS Professional learning & development reimbursement; CE credits through GGH Institute Collaborative and supportive community with dedicated care coordinators and MD advisors Malpractice coverage Diversity & Inclusion Greater Good Health is dedicated to building an inclusive, diverse, equitable, and accessible workplace.
