Occupational Therapist, Home Health

CenterWell Home Health is seeking a compassionate, patient-centered Occupational Therapist to provide one-on-one care in the home setting. In this role, you will deliver skilled occupational therapy services to adult and geriatric patients, focusing on improving safety, independence, and quality of life in their own homes. You will perform comprehensive evaluations, develop individualized treatment plans, and implement evidence-based interventions to address functional limitations in activities of daily living, mobility, cognition, and home safety. Working closely with the interdisciplinary care team—including nurses, physical therapists, speech therapists, social workers, and physicians—you will coordinate care, communicate changes in patient status, and contribute to patient-centered care plans. You will educate patients and caregivers on therapeutic activities, adaptive equipment, energy conservation, and home modifications to support autonomy and prevent rehospitalization. Documentation of assessments, progress notes, and discharge summaries will be completed accurately and in a timely manner, adhering to all regulatory, payer, and company standards.
