Job Title: Registered Nurse - Intensive Care Unit Profession: RN Specialty: ICU Duration: 13 weeks Shift: Night Hours per Shift: 12 Experience: Minimum 1 year License: Active RN Licensure Certifications: BLS, ACLS, NIH Certification Must-Have: Experience in critical care nursing Description: Medical ICU is a 20-bed mixed ICU/Step Down unit. It requires a combination of 2:1 ICU and 3:1 Stepdown staffing. Floating to a sister unit is required to care for Stepdown patients at a 1:3 or progressive level at 4:1. Patient population includes severe respiratory failure, such as pneumonia, ARDS, and COPD or asthma exacerbations, commonly requiring mechanical ventilation or high-flow oxygen support. Also includes sepsis and septic shock, often requiring vasopressors, invasive monitoring, and aggressive fluid management. Patients may present with hemodynamic instability, including hypotension, arrhythmias, or shock state. Neurologic conditions such as altered mental status, metabolic encephalopathy, overdose, or non-surgical stroke are common. Complex metabolic and endocrine disorders, like diabetic ketoacidosis, severe electrolyte abnormalities, or liver failure, are also managed. Cardiac conditions that do not require immediate intervention, including heart failure exacerbations, cardiomyopathy, or post-cardiac arrest care, are seen. Gastrointestinal and hepatic failure, including GI bleeding, acute liver failure, or pancreatitis, may be part of the patient population. Typical patient needs and acuity include continuous cardiac and hemodynamic monitoring. Invasive lines such as arterial lines may be utilized. NIH and TNK therapy are administered. Mechanical ventilation, noninvasive ventilation, or advanced oxygen therapies are common. Titrated IV medications such as vasopressors, sedatives, insulin, and antiarrhythmics are frequently used. Enteral or parenteral nutrition is provided as needed. Frequent laboratory monitoring and complex medication regimens are integral to patient care.
