Licensed Practical Nurse (LPN) - Long Term Care

Schedule

Full-time, Evening shifts, Every Other Weekend (EOW). Candidates must be available to stay through the full holiday season and start on a Tuesday or Thursday.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing care that is appropriate to the age of the patients served, demonstrating knowledge of growth and development across the lifespan. The LPN will provide direct and indirect skilled nursing care for assigned residents, adhering to policies and procedures under the supervision of a Registered Nurse.

Responsibilities

• Collect ongoing data relevant to the resident's physical and psychosocial condition, communicating findings to the RN and interdisciplinary team.
• Contribute to the development of age-appropriate, culturally relevant care plans reflecting nursing diagnoses and interventions.
• Recommend revisions to the plan of care based on evaluations of resident outcomes and indicate needs for referrals.
• Maintain current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.
• Ensure adherence to safety, cost-effectiveness, and impact in the delivery of care.
• Demonstrate knowledge and competency while providing leadership in the professional practice setting.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (AHA or RQI required)
• A minimum of 1 year of experience in an LPN capacity in a Long Term Care facility
