Physical Therapist

Schedule: 8-hour shifts

Assignment Details:

• Contract Length: 13 Weeks
• Guaranteed Hours: 40

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness for physical therapy.
• Document and complete a Plan of Care (POC), progress notes, and other clinical records. Revise the POC based on ongoing assessment data.
• Plan and provide treatment for various disorders based on medical referrals and therapy assessments.
• Assist and instruct patients and families on proper transfer techniques and the use of supportive equipment.
• Educate patients and families regarding therapeutic procedures tailored to the patient's condition and needs.
• Ensure the application of infection control and universal precautions in patient care activities.

• Requires the ability to work under stress to meet deadlines and patient needs.
