Growing together.The RN Supervisor oversees a caseload of 100-175 home care patients and provides direct supervision to clinical staff (RN, LPN, MSW and HHA) assigned to the team. The RN Supervisor is responsible for ensuring home care services are compliant with state and federal regulations regarding home care services and nursing practice. The RN Supervisor has significant and current clinical experience with triage skills and the ability to appropriately delegate and assign duties to staff. The RN Supervisor assures the RNs, LPNs, MSWs and HHAs under their supervision perform duties as stated in their respective job description.Primary Responsibilities:Ensures staff is compliant with Agency policies and standards, state and federal regulations through ongoing performance evaluation and management including, but not limited to, annual supervisor visits, routine case conferences, and routine 1:1 conferences. Serves as mentor and role model for staff by providing clinical and supervisory guidance and supportEnsures patient care is coordinated and documented effectively amongst disciplines through clinical record review and multidisciplinary case conferences. Assists with management of complex patient/caregiver situation by providing consultation to field staff to include home visitsEnsures patient care provided by team is coordinated to include staffing for day, evening, weekends, holidays and that team members participate in Agency on-call coverageConducts clinical field supervision and performance management monitoring of attendance, productivity, overtime, supplies and cost per visit to ensure compliance with Agency standards. Utilizes management reports and other resources to ensure ongoing management of patient outcomes and Team/Agency operational performanceAssumes individual responsibility for professional competence and professional nursing practiceProvides oversight of the plans of care for patients assigned to Team to include monitoring of complex care plans, documentation, and regulatory compliance review; utilizes key reports and clinical record review to ensure ongoing oversight of these key issuesCoordinates with RN Clinical Review Specialist regarding performance improvement activities including day-to-day operational issues and Team/Division/Agency initiativesInterviewing, selection and retention of qualified staff for the Team. Participates actively in the orientation of new organizational personnelAssists in the formulation, revision, implementation and evaluation of policies and procedures as well as strategic goals and objectivesPromotes positive customer service at all times both internally and externally in interactions with patients, families, referral sources, vendors, and co-workers of the team and other Teams/Divisions of Southwest Medical Home HealthTeam staff compliance with HR requirements (licensure, CPR, TB, policy and procedure review, competencies, etc.)Assists in the formulation, revision, implementation and evaluation of in-service and continuing education programsPrimary office environment, however, does make home visits in the community including supervisory visits and does participate in on-call rotation for weekend, night, and holiday coveragePerforms other duties as assigned by the Associate Director of NursingYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:High School diploma or equivalent Registered Nurse with active unrestricted license in the State of Nevada and a graduate of an approved school of professional nursingCurrent CPR certification (or obtain within 30 days of hire)2+ years of experience in community health or medical/surgical patient care setting, providing direct patient care1+ years of performance management with supervisory experienceExperience working with an electronic medical record application Demonstrates thorough knowledge of current nursing practice, professional standards of care, and state & federal regulations regarding home careBasic PC skills to include Windows, Outlook or other email systemHigh level of critical thinking skills to analyze multiple sources of information to formulate logical conclusions and actionsProven ability to plan, organize, prioritize and direct the work of others and communicate objectives clearlyProven excellent observation skills, verbal & written communication skills, attention to detail and demonstrated competence in core leadership skillsDemonstrated ability to successfully utilize clinical information systems for operation and performance management functionsValid Nevada driver's license and maintain personal auto insurance coveragePreferred Qualification:Bachelor's degree in Nursing or Healthcare related field or working towards completion of Bachelor's degreePay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $72,800 - $130,000 annually based on full-time employment.
