Key Responsibilities: Lead and coordinate the Individual Support Planning (ISP) process for residents in Shore’s Residential homes. Oversee the development, implementation, and monitoring of active treatment plans and program services. Ensure all services comply with state and federal regulations and organizational policies. Serve as a case manager, advocate, and liaison for residents, families, and service providers. Participate in a rotating on-call schedule to provide 24/7 shift support for residential homes. Collaborate with interdisciplinary teams to assess resident needs and adjust care plans accordingly. Document all services, interventions, and progress accurately and timely. Provide guidance and supervision to direct care staff regarding treatment plans and regulatory compliance. Skills and Qualifications: Bachelor’s degree in Human Services, Psychology, Social Work, or a related field. Valid certification as a Qualified Intellectual Disability Professional (QIDP) or eligible for certification. Prior experience working with individuals with intellectual and developmental disabilities preferred. Strong knowledge of state and federal regulations related to residential services. Excellent communication, advocacy, and interpersonal skills. Ability to work independently and as part of a team. Flexibility to work occasional evenings and weekends as needed. Valid driver’s license and reliable transportation may be required.
