Overview
Registered Nurse Case Manager (Travel) in Chillicothe, Ohio. Start date 09/21/2026 for a 13-week assignment. Weekly pay range: $2,376–$2,506 per week. Guaranteed hours: 40 per week. Shift: 5x8 Days. Setting: hospital-based case management for Acute Medical and Surgical patients. This assignment features dayshift hours with no on-call, holidays, or weekends.

Responsibilities
- Assess discharge needs, social determinants of health, and discharge planning/referrals for Acute Medical and Surgical patients
- Develop and implement safe, timely discharge plans and coordinate referrals to post-acute services
- Collaborate with an interdisciplinary care team to support smooth patient transitions
- Document care coordination activities in the patient record with accuracy and compliance
- Manage a typical caseload around 20 patients per day, ensuring timely discharge planning
- Communicate with patients, families, and care partners; provide education and resource linkage to support ongoing care

Requirements
- 3+ years Acute Care Case Management experience
- Must reside outside a 60-mile radius of the assignment location
- Ability to work in a hospital-based setting
- Strong communication and collaboration skills
- Experience with discharge planning and addressing social determinants of health
- Proficiency in clinical documentation and care coordination
